Job Description

Qualified Social Worker, Assistant Team Manager, Swindon Borough Council, Adults Hospital Discharge Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team

Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team

The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager

The responsibilities will include:

• Promote a culture of continuous improvement, through ownership of performance and maintaining quality assurance systems for the services
• Deliver the vision of the new Hospital Discharge Policy and support the team to provide operational cover 7 days a week, offering flexible working patterns over 7 days
• Work closely with acute and community health colleagues to ensure safe and effective discharges from the acute as well as working to prevent hospital admissions
• Liaise with other professionals
• 36 hours per week (flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance
